Meet Tara Salazar
Tara Salazar, MSN, RN, CCM, founded Medina Health Access Consulting to help close the gaps that people encounter across healthcare and community systems.
Her nursing and care coordination background spans acute care, case management, utilization review, behavioral health, and support for people managing chronic conditions. She brings a practical, person-centered perspective to improving how care is organized and delivered.
